1a) I don't know, but on an ocean voyage you would have lots of time to think about it. The obvious answer would be to poke a bit of line down through the hawsepipe, tie it on to the top link and so pull it up.
b) Ten years ago a big modern Lowestoft diesel beam trawler washed up onto the sands of the Norfolk coast in a rather embarrassing style because the engine failed and they couldn't get the chain out and fastened onto the anchor in time!

2a) I once came back from Portugal aboard the big sailing trawler 'Excelsior' with a broken engine crankshaft. The give the port authorities took a bit of persuading to allow us to sail engineless, but we got home perfectly well.
b)I have a friend who once built an ocean racer without an engine because he thought that this would give him a better rating - it wasn't even a nuisance for him since her engineless state was so well known that his competitors queued up to tow him whenever necessary.
c)I believe that there is a big clipper schooner in the US without an engine and that the Coastguards couldn't find any rule to prevent this.

I really question whether they deserve their cost and weight. And if you don't have one you have to think a bit more about your seamanship - which judging by the antics of so many yachts under power would be no bad thing.
