Cook could have dropped the plates carrying them from the scullery where they were washed to the pantry where they were stored. Or she could have been carrying the afternoon tea tray to or from the drawing room.

As for hatches. I don't know about common, but one of my grandparents' and my parents' house each had one as otherwise it would have been a long walk round from the kitchen to the dining room.
