As a writer, albeit not of AR's caliber, I can tell you that it can be quite difficult to manage a large number of characters and keep the story coherent. Could that be why so few have S,A,&Ds all together? AR does a fabulous job with managing his characters, but even he uses many tried techniques to accomplish it: breaking them up into smaller groups, letting characters fade into the background (Peggy!), etc. With ML, he introduces at least four new major characters integral to the story (M.Lee, Ama, Chang, ex-Cook) and several more minor ones (N&P's Captain, Wu, Councilor). And he already has a jolly time splitting the group. Adding the Ds might just have been more than the story could handle.

Or, the story he had in his head might simply have not included the Ds.

I agree that they're wonderful characters on many levels, though. Even if Dot does make me gnash my teeth at times...
