Some books eg Anthologies or Historical books with a variety of extracts or photos just say eg:

“The publishers have made every effort to trace the owners of these photos and apologise for any omissions”

(from “Station X: The Codebreakers of Bletchley Park” by Michael Smith; published by Channel 4, Britain)

I recall that for a while the heirs to Sir Arthur Conan Doyle prevented a couple of unpublished Sherlock Holmes stories from being published after his death, though they have since been published. I think that they were finished, but he originally decided not to publish them?

A webpage on US Copyright Law; apparently at one time you had to claim (and renew) copyright in America.
