I've just been rereading Swallowdale, and noticed that when Capt. Flint made his first appearance he asked Titty what Peter Duck had been doing since his return from the Caribees with his pockets stuffed with gold. Sounds like AR had the basic story line in mind then but hadn't worked out all the details since IIRC the treasure was pearls.
As to why Swallow has a patch on her sail, perhaps it's not the patch from the shipwreck (don't have a copy of PD; that and Missee Lee are at my parents) but from some other misadventure of the good ship Swallow. One of the chapter tailpieces in the beginning of Swallowdale shows what looks like a patch about 1/3 of the way up the leech. Of course with the dark sail in most of the pictures, a patch wouldn't show up too well if at all.
