Well, Harriet Vane smoked. Although whether she was "fast" or not seems to have depended a lot on one's biases. Somewhere in the 30s it appears to have become more acceptable for women to smoke (possibly due to heavy advertising) at least in the U.S.

WRT Inkings, smoking in The Lord of the Rings is almost considered an "art" (at least by the hobbits) but clearly isn't widespread and probably won't become that way (beer, on the other hand...)
