Even now my public school old boys' association still write to me as "Dear Roberts" - still it puts me in my place (which I probably deserve) especially when they are making another appeal for money.
Otherwise it is the more impersonal "Dear Old Boy".
Still I never had a forename at school only either a surname or nickname.
