Puffin Books, Harmondsworth, Middlesex, England

It is more normal to put a : between the Publisher and the location, also someone like Puffin Books will have an accepted acroynm (like OUP) as that is to long to type all the time, and no way anyone would type Harmondsworth, Middlesex, England - probably just Harmondsworth, as it would be a unique location for PB even if is a common town name like Oxford or Cambridge.
