I don't have the book to hand but I am sure it comes from very early in SA when they are making preparations for their first voyage to WCI.
Susan (who else?) worries about sickness and John comments that it is always the Mate who is the surgeon on board ship!! Mother decides that anybody who gets sick gets shipped home (Holly Howe) but Titty (who else!) says they can surely have a fever or two on their own!
