When a car engine is running at full load the car is travelling at high speed and there must be a massive air flow through the radiator. Its in traffic jams that the engine tends to overheat and it is running under little load. A marine engine is under full load at perhaps five knots.
One solution, particularly suitable for inland waterways, is keel cooling; a closed circuit system with the engine cooling water being passed through heat exchange pipes mounted on outside of the hull.
