Perhaps one could theorize that part of the reason AR anguished over his plots is because he couldn't focus as well on his writing due to his discomfort and pain.
Perhaps more important was that:
He needed the money, so the stories had to be good.
He was a writer through and through, and took pride in what he produced.
The children in his stories, and his potential readers, were a substitute for the children he and Evgenya wanted, so he really cared about the books.
Most likely a mixture of all of those. And remembering, too, that however self-confident creative artists may appear to be, they are always in a terrible stew about what they are doing.
