It's not strictly related to "had had", but I can't resist passing on a favourite item of syntactical usage. It concerns a little girl whose mother was about to read her a bedtime story. But mother had brought up the wrong book, and her daughter said, "Oh Mummy, what did you bring the book I didn't want to be read to out of up for?"
