Yes, Ed, it certainly looks very dangerous. I think, though, that there has been a certain amount of artistic licence in the matter of the flames. One would normally cook over hot coals, much as one would on a barbecue. I also think that the kettle shown would normally be slid to one side, perhaps with a stout stick, before lifting the lid with a potholder in the hand. The fireplace in my grandparents' cottage had a swinging arm as you have described, which make the cooking easier. Still not easy, but easier.
