I really don't think any of these measurements are critical. If the punch will fit over the head of the nail it will do the job.
All that is needed is a hole drilled in a piece of steel bar about 4" long. Very easy with a lathe, but I imagine it would be perfectly practicable to do it with a hand drill, holding the bar in a vice.
The point about clinker, as a thousand year old craft, is that it isn't high precision work. The Vikings did it with whatever was to hand, and so can you.
