Certainly Miss and Mr were used in letters even in my childhood, but if that had just been the case in S&A, wouldn't Nancy & Peggy have called him Mr Sammy?
In later books the children are all called Mr or Miss by the local people - Swallowdale with the Swainsons and the Dixon's & Jackson's in Winter Holiday.
A very interesting book 'Debs at War' by Anne de Courcy(Weidenfeld & Nicolson, 2005) gives a great insight into class distinctions pre and during WW2. The experiences of nearly 50 young women show how their births influenced the way they were treated (good & bad) during the war.
