Some writers write the dark side, some the perfect side of life. Even though he had created a relatively good life for himself, possibly he was creating his perfect life, warts & all, remembering his childhood and watching the childhood of all the children he associated with to create a pastiche in his mind.
Re; illness - its also very personal as to how well you can create when your health is less than normal. Some do it and work through it, some get distracted or the muse packs a sulk, and yet others drink or drug themselves to get through it. Sometimes you can do technical work (editing and drafting) when you aren't feeling well and the creative stuff is left for better days.
