When Roger hurts his foot in SD, he spends a whole evening with Young Billy, who never calls him anything but "lad". In the same context, he calls Titty "miss". This could be nothing to do with manners, but simply because YB couldn't remember their names - he'd only met them once the previous year during S&A.
