Owen, Wayne Hammond's Bibliography mentions the 2-page summary in the first edition, and comments (p.91):
The author's note, pp.13-14 in the original edition, was later omitted.
So Wayne has not seen enough reprints to trace the date. Since on the previous page he says that textual changes were made
some time after the third printing (1933) but no later than 1936
it would suggest the summary went at the same time. That would fit in with your 1937 printing.
Wayne describes p15 as having the single word SWALLOWDALE, and p16 has the Ship's papers. My 1941 printing has SWALLOWDALE on p13, the Ship's papers on p15, and pp 14 and 16 blank. Yours is probably the same.
I was utterly baffled by Peter Duck when I first read SD, aged 7, before reading PD. If only thaose two pages had still been there.
