I am not against realism but these passages merely grate.

I think I have always viewed these passages somewhat differently.
The fact that the twins are not sniggering behind their hands, but talking "Ginty" to her face and sometimes in the presence of their father points to a robust, even loving, relationship rather than one of disrespect.
I also feel that the twins' comments in Ginty do add humour to the book.

I don't see Susan's comment as disparaging. More likely evidence that she was very worried about their fate. Susan was a worrier usually from the best of reasons, but still a worrier. The fact that on the surface it seems "out-of-role" to me demonstrates the level of her anxiety.
