Oh dear - Much as I dislike saying this, the evidence is stacking up in favour of the 'class' interpretation, but ee it's complex. People like Cook, the postman, Slater Bob and Mary Swainson say 'Miss Nancy'. The doctor says just 'Nancy'. Cook says 'Mrs Lewthwaite', but says 'Sammy'. Mrs Tyson says 'Miss Nancy'. Mrs Dixon says 'Miss Ruth'. Nancy calls Mary Swainson 'Mary' but this is in her absence. The G.A. calls Col Jolys 'Tommy Jolys', while he and the police sergeant call her 'Madam'. Captain Flint addressed Col Jolys as 'Jolys'. Jacky and the D's, refreshingly, did not call each other anything.

Because the Amazons are female, AR did not have to work out how natives would have addressed a middle-class boy. He took care not to have any of the locals address John or Roger. One assumes they would have said 'Master John'.

The heck with this . . .

The one consolation was mentioned by Adam. Nowhere in all this is there any absence of respect, no matter who is addressing whom (with the one exception of the G.A.'s 'Tommy!').
